Which part of Joomla / component to look at for altering native SEF

I am quite a fan of K2 these days. One tiny niggle is its native SEF. This is one aspect of Joomla that I am not clear on. K2 has its own router file which is as I understand it where it builds its SEF url. However I have never liked the word 'item' appearing in the url itself. I'd rather see just the category/article

http://www.example.com/[menualias]/[category]/[article]

instead we have

http://www.example.com/[menualias]/[category]/item/[article]

So my question is how do I get rid of the item from the URL in a sensible way? There doesnt seem to be a natural solution in htaccess and i dont want to limit the solution to LAMP platforms. 3rd party SEF drives me nuts with its over zealous  production of 404s and I'd rather stick with the core sef.

So where to start? Any tech steers welcome :)
